Service Coordinators are a valuable member of our core team. They typically have social work or human services education and experience. The role of the service coordinator is to link elderly, disabled and low-income families to supportive services and other community resources.
Service Coordinator Responsibilities/Requirements
Coordinate the consumers' eligible services with providers, submit and advocate service plans to Office of Long Term Living (OLTL) and/or Managed Care Organizations (MCO) for approval, and ensure the delivery of all services. Coordinate with all parties involved in the consumer's care, including but not limited to, POA/guardian, representatives, providers, vendors, nurses, physicians, Maximus, Area on Aging, County Assistance Office, OLTL to ensure consumer receives proper care. Service coordinators advocate for consumer needs through peer counseling, providing information and referrals for services, and collaborate with the consumer to develop and complete their person centered plan of care.
